What is Levator Ani Syndrome and Proctalgia Fugax?

Levator Ani Syndrome (also known as levator syndrome and proctodynia) is rectal pain that is caused by spasm of the levator ani muscle. The true origin is not known.

Common symptoms include a dull ache high in the rectum and a feeling of constant rectal pressure, burning, or tightness. Many have said it feels like a "golfball in the rectum". The pain can also be felt all throughout the pelvis and perineum.

Relief can sometimes be found by walking or consistent stretching. Other treatments include massage of the deep pelvic floor muscles digitally, warm sitz baths, muscle relaxant type medications, and biofeedback. Electrical stimulation (E-Stim) of the levator ani muscle has been used to break the spastic cycle of the levator ani muscle. Injections of botulinum toxin A has also been used in many cases with varied success.

Many others have had success approaching Levator Ani Syndrome as a mind-body disorder in the form of TMS or Tension Myositis Syndrome. More information can be found on this online and within the forum.

Variants of levator ani syndrome includes proctalgia fugax (fleeting sharp pain in the rectum) and coccydynia (pain in the coccygeal region near the tailbone). Stress seems to be a common trigger in many cases of both levator ani syndrome, pelvic pain, and proctalgia fugax.
